How Initiative Works in 5e
When combat starts, every participant rolls initiative: a d20 plus their Dexterity modifier. Everyone acts in descending order of their results, and the order repeats each round — one round representing six seconds in the game world. The tracker sorts combatants automatically as you enter their rolls, lets you break ties by reordering manually, and highlights whose turn it is so nobody at the table has to ask.
Tracking HP and Conditions
Each combatant card tracks current and maximum hit points with quick damage and healing entry, plus temporary HP handled separately as 5e requires. All standard conditions — blinded, charmed, frightened, grappled, poisoned, prone, stunned, and the rest — can be toggled on any combatant so you never forget the wizard is still concentrating or the ogre is prone. A notes field per combatant covers everything else, from death saves to legendary resistance counts.
